End-of-life care
Caring for somebody approaching the end of their life is challenging and emotionally demanding, but also one of the most important and rewarding areas of what we do.
Our services are fully centred on the person who is approaching the end of their life, ensuring that their needs and wishes are communicated, recorded and form the core of their care plan. The focus of all our care delivery is the comfort, well-being and symptom management of the individual whilst taking into full consideration their spiritual, cultural and psychological needs and right to choose how they are cared for and where they wish to die.
We work closely with medical professionals, other organisations (e.g. Macmillan nurses), Social Services and family member to ensure that a coordinated, multi-agency approach is maintained and that care can be delivered effectively and cooperatively.
Involving family, close friends and informal carers in decision making as well as recognising their own needs is essential to providing a comprehensive package of care.
We also recognise the importance of emotional and psychological support for family members and carers and offer advice and training to help deal with this particularly in the last days of life and after bereavement.
